Activity Notification Form

This online form must be submitted at least 7 days before any activity. Choose your District (or County if relevant) as you complete the form, this will then be automatically sent to your District Lead Volunteer. A copy will also be sent to you for your records. Please ensure your Group Lead Volunteer (GLV) is also made aware of the activity.
